Accident investigation process
All accidents, injuries, dangerous occurrences, and near misses should be reported and impartially but thoroughly investigated. Risk Assessment Solutions Limited offers a full accident investigation service to our clients. We endeavour to:
• Reach the scene as quickly as possible
• Record the scene with photographs or video
• Leave the scene unaltered for as long as possible
We will use the technical and practical experience of our trained personnel to establish the WHAT, WHERE, HOW, WHEN and WHO by:
• Interviewing and taking statements from the injured persons, witnesses and others
• Evaluate documents including maintenance and training
• Offer an interpretation of Health and Safety Law
• Present conclusions as to the remedial action required
The report that you will receive as a Manager or Director will:
• Establish all the relevant factors as quickly as possible - particularly the causation and sequence of events leading to the accident/incident
• Present interview statements from all who appear to be able to contribute to the investigation.
• Eliminate speculation and conjecture
• Evaluate all the facts as to the accuracy, reliability and relevance
